• Remember that copays and any outstanding balances are due at check-in

Make sure to bring the following to your first appointment:

• Insurance card and copay

• List of current medications

• $4 for parking (credit card only)

• Current glasses or contact lens box (if new prescription needed)

• Someone to drive you home (if having a procedure or dilated exam)
